2 months well child care visit: Vaxelis (DTaP-Polio-Hib-Hepatitis B), Prevnar (pneumococcal disease), Rotateq (rotavirus)
4 month well child care visit: Vaxelis, Prevnar, Rotateq
6 month well child care visit: Vaxelis, Prevnar, Rotateq
12 month well child care visit: MMR, Varivax (chicken pox), Hepatitis A
15 month well child care visit: Daptacel (DTaP), Prevnar, ActHib (Hib)
18 month well child care visit: Hepatitis A
4 year well child care visit: Proquad (MMR - Varivax), Quadracel (DTaP - Polio)
11 year well child care visit: Menactra (meningococcal disease), Adacel (Tdap), and Gardasil (3 dose schedule for Human Papillomavirus)
16 year well child care visit: Menactra (dose 2), Meningococcal B (2 dose schedule)
